Employers should report an injury to MEMIC as soon as is possible when you have been given notice or have knowledge of an employee's injury. All claims should be reported to MEMIC within 24 hours of your notice or knowledge of the injury. 

You can choose one of four ways to report an injury, including MEMIC’s Injury Reporting Portal.

  1. Submit an online injury report
  2. Submit an injury report by phone 
  3. Submit an injury report by fax (fax injury report form is available from your state’s workers’ compensation agency)
  4. Mail an injury report form to MEMIC within 24 hours

When you report an injury to MEMIC, you’ll need the following information.

  • Your MEMIC policy number
  • The injured employee's name
  • The employee’s demographic information (social security number, date of birth, address)
  • Date of injury
  • Date you were notified of the injury

After an injury is reported, the employee will be assigned a claim handler. Typically, each state sets a time frame on the number of days MEMIC has to conduct a thorough investigation of the claim before it makes a decision to either pay or deny the claim.
